In my practice setting as a nurse, I commonly perform comprehensive nursing health assessments. These assessments involve gathering detailed information about the patient's medical history, symptoms, lifestyle, and other relevant factors. The assessments are focused on identifying any present health issues and determining the appropriate course of action for treatment.
Some examples of subjective data collected by nurses during health assessments include the patient's chief complaint, medical history, allergies, and any current medications they may be taking. Objective data collected may include vital signs, physical examination findings, laboratory results, and diagnostic test results. By gathering both subjective and objective data, nurses are able to form a comprehensive picture of the patient's health status.
The typical patient population in my practice setting consists mainly of middle-aged and elderly individuals. However, we also see a significant number of younger patients with chronic health conditions. Special considerations have to be made while obtaining an accurate health history and physical assessment in this patient population. For older adults, we need to take into account age-related changes such as reduced hearing or cognitive function. With younger patients, we may need to consider factors such as lifestyle choices, financial status, or cultural beliefs that may affect their health status and treatment options. By being sensitive to these special considerations, we can ensure that our health assessments are accurate and tailored to each patient's specific needs.
